All medicals MUST be completed before you go to the pool.
Although the pool is shallow (3 metres) CMAS insist that the club take reasonable precautions.
The blank medical form can be found in your course pack
It is in the small white booklet on the right of the folder.
Alternatively, you can use the CMAS web site. If you would prefer you can download a blank CMAS
medical form from the web.
Go to documentation (top line); CMAS-ISA General documents; Document # 7 (CMAS-ISA Diver medical form);
Print.
If you do not have a medical then you CANNOT dive.
The medical form should be taken to a GP or diving physician.
I prefer you to go to a dive physician, they are regular doctors who specialise in hyperbaric medicine
They are much more thorough, however, a non dive doctor is adequate
If you intend to pursue 2* (30 metres) it is wise to get an X ray of the chest
If you do not intend to dive deeper than 18 metres an X ray is probably not worth the money at this stage
Most dive physicians insist on a chest X ray
The dive medical must be signed, dated and stamped by the clinician (not you) at the bottom.
The medical is to ensure you are physically fit to dive at depth.
It is for YOUR safety and the safety of your buddy that we insist on the medical.
The medical is valid for two years.

A photocopy of your medical form will be attached to your membership form and the details
lodged in the bottom clubhouse.
